What is continuing care (aftercare)?
Continuing care, often called aftercare, is the ongoing support that keeps recovery on track after you finish detox and residential treatment. It includes a personalized step-down plan, ongoing check-ins, medication support, family involvement, and connection to a recovery community. The weeks and months after treatment are when relapse risk is highest, so a strong continuing-care plan is one of the best predictors of lasting sobriety.
